Thank you for coming back to see us. We ask that you include your top 3 appointment request times below and we will make every effort to accommodate your top request. We typically see patients in the office every Tuesday and Thursday between 7:30am and 4:00pm. Those would be our preferred times to visit. However, we understand that these times may not work for everyone and would like to provide you with an opportunity to see us that works best for you. This could be at any time or day, perhaps before or after work or normal hours, or even online. Please submit your requests below and you will hear from us within 24 hours. You may also directly request an appointment using your Klara account. If this is an urgent visit or a same day visit is requested, please call the office at . If this is an emergent situation, please proceed to the nearest Emergency Room.
If your visit is an MRI follow up, PLEASE DO NOT FORGET TO BRING THE DISC INCLUDING THE IMAGES as well as the RADIOLOGIST REPORT. Your personal health records are unfortunately not accessible from all electronic platforms and you physically bringing the information is best. Appointment requests should also be made for only one body part/region at a time. Focusing on one area per visit gives us time to really get to know you and understand your orthopedic problem. Please also try to arrive at least 10-15 minutes before your scheduled time, especially if you need to fill out paperwork. If you are running late or need to reschedule, no worries, please just call to let us know. Dr. Jaglowski cares greatly about minimizing wait times and reducing the normal hassles surrounding medical care and you are a vital part of that process as well.
